We're going to take a little time to come up with some good example spells, but in the meantime we can answer your other questions.
As covered in another question's answer here, if a character is explicitly retaining their previous boons, then no adjustment is necessary -- however, all the previously established limits of that system would still apply. This includes time limits of transformation, number of uses per day, and so forth. If you need a reminder of what those limits were, we have the old version of our boons page saved, and we can copy any relevant information to you.
Since the new system does not have the same stringency with regards to fog form transformations, for example, if you wished to take advantage of the new freedom for transformations (time/duration/type of creature), your character would need to progress to the appropriate tier. In short, if your adept wants to keep their old fog form, they can stay at Tier 2, but if they want the advantages of the new, alternate form, they would need to reach Tier 3.

Regarding Reira: judging by your description of his actions, as his subsequent works for the god have been both unintentional and of a lesser "weight" than some other methods, they would not be enough to qualify him for Tier 3 at this time. (For instance, if he had "accidentally" killed six Fog followers in one go, that sort of "unintentional" service would qualify him for promotion.)
Eagerness, however, is to be rewarded: in the near future, the Fourth god may give Reira a specific task or tasks to complete. If he does so successfully, this could be submitted for Tier 3 consideration.
